Vocational Rehabilitation Services Application Instructions
Please read the "Consumer Information Handbook" (in Rich Text Format) for an explanation of Civil Rights, Eligibility Requirements, Confidentiality, Appeals Procedure, and the Client Assistance Program before proceeding. After reading the "Handbook" if you feel that you qualify as a DOR consumer and you believe that the Department can assist you in achieving your employment goals, then do one of the following:
- Online Application: Apply on our website. Your application will be automatically sent to the office nearest you and you will be contacted by a Rehabilitation Counselor to schedule an appointment.
- Printer Friendly: Use one of the
printer friendly forms,

complete it, then locate the nearest Department of Rehabilitation office and- Mail the application to the office and they will contact you for an appointment with a rehabilitation counselor,
- Phone the office for an appointment with a rehabilitation counselor and bring the application with you to your appointment, or
- Bring the application to the office and they will schedule an appointment with a rehabilitation counselor for you.
If you need assistance completing the application, the rehabilitation counselor can assist you.
