Notice of Central Office Relocation

As of June 20, 1998, the address of the Central Office of the California Department of Rehabilitation will be:

2000 Evergreen Street
Sacramento, CA 95815

Central Office personnel include the Director's Office, Administrative Services Division, Field Operations Division Central Administration, Independent Living and Technology Division and Program and Community Services Division.

New phone numbers will be in effect after June 20. Referrals to the new numbers will be available by calling the old phone numbers.

The department's post office box will not change.
